When booking, please have your insurance card and any referring provider information available
What to Bring to Your Visit
- Government-issued ID
- Insurance card(s)
- List of current medications (including dosages and supplements)
- Relevant medical records, test results, or imaging
- Copay or payment method
- Referral or authorization (if required by your plan)
Before Your Appointment
To save time, you can complete your forms before your appointment.
If you’re unsure which forms apply to you, call our front desk. We’re happy to help.
After Your Visit
You can view results, send messages, and pay bills through our Patient Portal.
If your provider orders additional testing or follow-up appointments, our staff will help you schedule before you leave.
